Pre Session Questionnaire

These adaptive gaming session questionnaires were created to help the GAME Checkpoint Leads collect the information prior to a gamers first session.

  • The goal is to understand the gamers goals, determine the type of AT to try in the first session, and to help document the process while working with a gamer.
  • The content in the questionnaire covers the gamers goals, physical and cognitive abilities, and environment/support information.

There are three documents you can download that are explained below:

Gamer Questionnaire - Insights

This document includes all of the questions for the users along with notes that MMC staff have added to give the GAME Checkpoint Lead more context to “why” the question is being asked.

Gamer Questionnaire - PDF Fillable

This document is intended to be sent directly to gamers or to be filled out. All of the content from the same questions from the insights document but without the notes for the GAME Checkpoint Lead.

This is the Word doc version of the “PDF Fillable”. You can use this if you want to use the questionnaire as a template and change any information or if it is easier to fill out using Word.
