Session Questionnaire

These adaptive gaming session questionnaires were created to help the GAME Checkpoint Leads collect the information prior to a gamers first session.

The goal is to understand the gamers goals, determine the type of AT to try in the first session, and to help document the process while working with a gamer.

The content in the questionnaire covers the gamers goals, physical and cognitive abilties, and environment/support information.

This document includes all of the questions for the users along with notes that MMC staff have added to give the GAME Checkpoint Lead more context to “why” the question is being asked.
